Application Documents:
- OPTIONS Application (online secure form): We strongly encourage all applicants to attend a brief evening information session or schedule a daytime appointment with an OPTIONS representative to review the unique format of our programs and ensure it's a good fit for you,

OPTIONS Application (pdf). - OPTIONS letter of recommendation (pdf): You will need to obtain two recommendations from professional references qualified to judge your expertise, character, and capacity for college learning.
- OPTIONS transcript request (pdf): To request your transcripts from other institutions, complete this form and mail or fax directly to school(s) attended with their appropriate transcript fee.
