The following is a list of Financial Aid forms.

Please complete these forms ONLY when requested by your Main Campus financial aid advisor.

Once you have completed the requested form(s), please print the form(s). sign and return the form(s) to the Financial Aid Office.

  • Access Missouri Statement of Understanding: You have been awarded the Access Missouri grant that has certain eligibility requirements.
  • Asset Form: This form is requested when questions 40-42 or questions 88-90 are left blank on your FAFSA
  • Dependency Status Form: This form is requested when any of questions 45-57 are left blank on your FAFSA
  • Family Budget Worksheet for Independent Students: Per Fontbonne University policy, we must ask single students making less than $15,000/yr and married students or students with dependents making less than $25,000/yr how they were able to support themselves financially.
  • FERPA: Federal Educational Rights and Privacy Act Privacy Act. FERPA is a federal law that protects the privacy of student education records. This form gives the Financial Aid office permission to discuss your financial aid with any of the people you list on this form.
  • Financial Information Form: This form is requested when questions 43 & 44 or questions 91 & 92 are left blank on your FAFSA.
  • Independent Verification Form: This form is used for independent students who have been selected for verification from the Department of Education.
  • Parent PLUS Request Form: To be completed by parent if parent is taking out a PLUS loan.
  • Parent/Student must eSign FAFSA. Parent/Student failed to electronically sign the FAFSA using the four-digit PIN issued by the Department of Education. You must go to www.fafsa.gov and eSign the FAFSA using your PIN. If you have lost your PIN, go to www.pin.ed.gov to have your PIN displayed or to have a new PIN generated. This may take up to three days from the date of the request.
  • Professional Judgment: Complete this form only when requested by your financial aid counselor.
  • Student must update FAFSA using IRS Data Retrieval Tool – Once a student has filed their federal tax return with the IRS, they may be eligible to use the IRS Data Retrieval Tool within the Free Application for Federal Student Aid (FAFSA) at www.fafsa.gov. With just a few simple steps, a student can view information from their IRS tax return and transfer that information directly into the FAFSA.   • Tax Exemption Clarification for Independent Students: According to information you provided on your FAFSA, there is a difference in the number of people in the household AND the number of exemptions claimed the Federal Tax Return.
  • TEACH Grant Statement of Understanding: You have been awarded a TEACH grant that has certain eligibility requirements.
  • Verification of College Attendance and Household Size: You must confirm the number in college or the household size.
  • Verification of Student Separation or Divorce: Complete this form and return with requested documents.
