As of January 26, 2007, 1098-T for 2006 tax year have been mailed. If you would like to request an additional copy of a 1098-T please complete the request form below. Once you have submitted your request you will be redirected to receipt of submission page.

Forms will be processed within 7-10 business days from the time of the request.

To view your 2006 statement, please visit our website at www.fontbonne.edu. From the Fontbonne homepage go to the shortcuts dropdown box and click on current students located in the upper right corner. Next click on "Student Information Managment" link located at the top of the list. Follow the directions on the page to log in. From the Student Information Managment page, select the plus sign next to the Business Office or Accounting located at the bottom left side of the page. Select 1098T from the drop down menu.
